Reporting a death
Letting us know when a loved one has passed away
Firstly, we’re very sorry for your loss. We understand this is a difficult time, and our team will do everything they can to make things as simple and stress-free as possible.
If the person who has passed away held one of our policies, we ask that you download and complete the Death Claim form. Once completed please return to us along with the required documentation (see FAQs below for more information) by using form below, emailing insure@unitymutual.co.uk or by post to the below address:
Freepost UNITY MUTUAL (please ensure "UNITY MUTUAL" is written in capital letters).
Once we’ve been informed, we’ll begin the process of calculating the value of the policy.

Protecting your loved one’s identity after they’ve passed away
Sadly, fraudsters sometimes try to use the personal details of someone who has died to commit identity fraud. We understand how upsetting this can be for families.
To help protect your loved one’s identity, we recommend using the below services to remove their details from mailing lists and official records. This reduces the risk of their information being misused.
FAQs
To help us begin processing the policy, it’s helpful to have the following documents ready:
- The original or a certified copy of the death certificate
- The Last Will and Testament
- Grant of Probate or Letters of Administration
- The original policy document
If the original policy document is missing, please let our team know and provide any details you have about the policy e.g. policy number, type of policy, or when it was taken out.
Our team will use this information to help locate the policy and guide you on the next steps.
We usually require originals or certified copies of legal documents. If you're unsure about what's needed, please contact a member of the team and they’ll guide you through it.
